Local / National Numbers

  • Letter of Authorization (LoA) — Porting wish date must be mentioned; the signature must be a wet signature (no electronic signatures)
  • Latest Invoice — Most recent billing documentation from current provider
  • VAT / TAX ID — Tax identification number of the requesting entity
  • Proof of Address — Dated within 3 months of the request (acceptable: latest invoice, utility bill, or insurance document)
