All Number Types (Local / National / Toll-Free)

• ☐ LOA (Letter of Authorization) — Must include a local Danish address. A wet signature (handwritten, not digital) is mandatory. A downloadable template is available — contact support@blazesms.com to obtain it.
• ☐ Local TAX ID / VAT — Danish tax identification or VAT registration number
• ☐ Latest Invoice — Most recent billing statement from the current carrier

Restrictions & Special Conditions

• Local Danish address required — The account holder must have a valid address within Denmark. This is a non-negotiable requirement for all port types.
• Wet signature on LOA — Digital or electronic signatures are not accepted for Danish porting requests.
