Local / National Numbers

• ☐ Letter of Authorization (LOA) — Downloadable template, must be completed and signed
• ☐ Latest invoice — Current billing documentation from the existing provider
• ☐ ID number of the authorized person — Personal identification of the representative handling the port
• ☐ Company Registration number — Required only if the number belongs to a business entity

• Ensure the authorized person's ID number matches the account holder or an authorized representative
• Business ports require the additional Company Registration number; personal ports do not
