Portable Number Types

Required Documents

All Number Types (Local / National / Toll-Free)

• ☐ LOA (Letter of Authorization) — Must include a local service address. Contact support@blazesms.com to obtain the Czech Republic-specific template.
• ☐ OKU (Migration Code) — Authorization code required for the porting process; obtain this from your current carrier
• ☐ Latest Invoice — Most recent billing statement from the current carrier
• ☐ VAT / TAX ID — Tax identification number of the account holder
• ☐ Proof of Address — Residential or business address verification

Porting Process

1. Contact support@blazesms.com to request the Czech Republic LOA template
2. Obtain the OKU (migration code) from your current carrier
3. Complete and sign the LOA
4. Submit the LOA along with all required documents (invoice, VAT/TAX ID, proof of address, local service address)
5. BlazeSMS processes your port request

• The LOA template is not available for direct download — you must contact support@blazesms.com to receive it
• The OKU migration code is unique to Czech porting regulations and is mandatory
