• ☐ Letter of Authorization (LOA) — Downloadable international template, must be downloaded and completed
• ☐ VAT/TAX ID — Business tax identification number
• ☐ Local contact phone number — A valid contact number within Luxembourg
• ☐ Latest invoice — Recent billing documentation from the current provider
