- ☐ Latest Invoice — Most recent billing documentation from current provider
- ☐ Copy of Authorized Person's Passport / ID — Government-issued identification
- ☐ Number Type Designation — Must specify whether the number is AG, PBX G, or IN
- ☐ Proof of Address — Document verifying the address associated with the number
